GLL CERTIFICATION

PRYAA and the teams we play against are members of the Gwinnett Lacrosse League (GLL).  For the safety of all players, the GLL REQUIRES EVERY SINGLE PLAYER FOR SPRING LACROSSE BE PHYSICALLY AND GRADE LEVEL CERTIFIED.  

PLAYERS WHO DO NOT COMPLETE CERTIFICATION ARE NOT ALLOWED TO PLAY.

The GLL Certification Process has 3 parts:

(1) 2023 GLL Physical

• Physicals must be good through May, 2023.  Partial season physicals must have an updated physical replace it before the old one expires.
• Please print the 2023 GLL Physical form and have your physician's office fill it out. 
• Make sure all fields are completed:
  • Legal name
  • Date of Birth (DOB)
  • Parent/guardian verifications and signature/date
  • Physician signature/date
  • Doctor’s office stamp or written address
  • Player must be cleared for all sports.  Any reservations and/or explanations must be noted.

If you have a physical for another sport in the last six months you may use it.  However you must also complete the top of the 2023 GLL physical form, and both forms should be uploaded together.

(2) Proof of Grade Level

Grade verification document must show on one document:

• Player’s first and last name (matching physical form)
• Grade level (example 7th grade, not the player's classroom grades)
• School year (2022-2023 school year).

Please send a photo of a current grade report/report card, or a document given to you by your school, or a screen shot from a school application which includes the required data.

For students in Gwinnett County Public Schools, an easy place to screenshot this information is on the ParentVUE site, or the StudentVUE site if you and your player do not have the same last name.

Homeschool families - please provide a copy of the homeschool letter of intent you filed with the GADOE and your child’s birth certificate.

(3) GLL Certification Website - please go to GLLcert.com

• Create an account or login to a previously created account
• Please enter each player’s full legal first and last name, so all documents will match
• Review and complete the form, including uploading a copy of the physical and the grade level proof noted above
